Let's get you started. The following things are REQUIRED of all new members:
New Member Affiliation Form, this will add you to your chapter's roster
The Greek Officer Contact Form is required to be completed by every chapter by the end of every fall semester or when more than five (5) officers change. The Individual Greek Officer Contact Form is due whenever five (5) or fewer officers change. To update advisor, headquarters, or housing board contact information, please email your FSP liaison.

- Only can be used if updating 5 or fewer officers
At the beginning of each semester the following forms need to be completed. They are due 3 weeks after the first day of school.
- The University of Arizona & Fraternity/Sorority Relationship Statement
- Hazing Compliance Form (Needs to be printed and signed)
- Indemnification Form
- Intake Declaration Form (USFC Only)
