Let's get you started. The following things are REQUIRED of all new members:
New Member Affiliation Form, this will add you to your chapter's roster
The Fraternity/Sorority Relationship statement is reviewed annually at the chapter president's onboarding.
These forms are due whenever there are changes in officer contact information. To update advisor or housing contact information, please email your FSP liaison.
- Greek Officer Contact Update Form
- Individual Greek Officer Contact Update Form
- Only can be used if updating 5 or fewer officers
At the beginning of each semester the following forms need to be completed. They are due 3 weeks after the first day of school
- Hazing Compliance Form (Needs to be printed and signed)
- Indemnification Form
- Intake Declaration Form (USFC Only)
